Personal data includes name, address, telephone number, email address, and date of birth (where applicable). The Centre will only request your personal data when conducting specific activities, such as promoting courses and events, conducting research, and carrying out surveys. The data will only be used for the designated activity purposes; the Centre will not disclose any personal data to external parties.<br \/>Before collecting personal data from individuals, the Centre will provide them with a Personal Information Collection Statement in an appropriate form and manner (for example, including the statement on application forms or providing it on the website, and displaying relevant notices at the reception areas of all its centres).<\/p><p><strong>Practices Statement<\/strong><\/p><p>The categories of personal data held by the Centre are recorded in the following records:<\/p><ul><li>Personnel Records: These include job application forms, personal data of employed staff, job details, salary information, payment records, benefits, leave and training<br \/>records, insurance records, MPF contributions, performance appraisals, and disciplinary matters.<\/li><li>Children&#8217;s File Records: These include the personal data of the child and the data of the parent, guardian, or carer providing the information.<\/li><li>Other Records: These include administrative and operational files, personal data provided by service users participating in seminars or workshops organised by the Centre, records of educational and training activities conducted by the Centre, etc.<\/li><\/ul><p>\u00a0<\/p><p><strong>Main Purposes of Keeping Personal Data<\/strong><\/p><ul><li>Personnel Records: The purpose of keeping employee personal data is for recruitment and human resources management, such as matters related to the<br \/>employee&#8217;s appointment, benefits, termination, performance appraisal, and discipline.<\/li><li>Children&#8217;s File Records: The purpose of keeping this data is to provide appropriate services related to assessment, therapy, or training activities, offer advice, or for<br \/>reference and follow-up by relevant parties when necessary.<\/li><li>Other Records: The purposes for keeping these records vary depending on their nature, for example, administrative work related to activities such as seminars or workshops organised by the Centre.<\/li><\/ul><p>\u00a0<\/p><p><strong>Security Measures<\/strong><\/p><p>The Centre takes appropriate steps to protect the personal data it holds from loss, unauthorised access, use, modification, or disclosure.<\/p><p><strong>Data Retention<\/strong><\/p><p>The Centre maintains and enforces a retention policy for records containing personal data to ensure that personal data is kept for no longer than is necessary. Different<br \/>types of personal data collected and held have different retention periods. Please refer to the Personal Information Collection Statement for the respective services.<\/p><p><strong>Disclosure of Personal Data<\/strong><\/p><p>The personal data collected by the Centre is provided voluntarily by service users, including name, company name, address, telephone number, email, etc. The<br \/>collected personal data will only be used for purposes directly related to administrative functions, service delivery assessment, therapy, or training activities.<br \/>Therefore, the relevant personal data will only be transferred to other relevant individuals or organisations with the consent of the participating service user.<\/p><p><strong>Access, Correction, Updating, or Deletion of Data<\/strong><\/p><p>If you need to access, correct, update, or delete any of your personal data, please notify the Centre by email, fax, telephone, or post.<\/p><p><strong>Data Access Requests<\/strong><\/p><p>Any enquiries regarding the Centre&#8217;s personal data privacy policy can be directed to Manager, Grace Tai, during office hours at 2708 8768. To make a data access request to the Centre, please complete the &#8220;Data Access Request Form&#8221; of the Office of the Privacy Commissioner for Personal Data, Hong Kong, and return it to the Centre together with an application fee of HKD $500. The Centre will verify the identity of the requester when processing a data access or correction request to ensure they are legally entitled to make such a request.
